365CGM.com Terms of Service

1. Overview
These Terms govern your access to and use of 365CGM.com. This is a personal-use software service website for viewing glucose readings, related events, and optional Nightscout-synced data. It is a secondary-data dashboard and a data-transfer utility. It acts as a mirror for records retrieved from the Senseonics Holdings (Senseonics) data servers for the FDA-approved medical device, the CGM Eversense 365. It has no understanding of, or accuracy in, the data it displays or transfers. It does not provide medical advice, diagnosis, or treatment. Nothing on 365CGM.com is a substitute for Senseonics's products, professional medical judgment, manufacturer instructions, or advice from a licensed clinician.
By using 365CGM.com, you agree to these Terms.
2. Eligibility and invited access
You may use 365CGM.com only if:
- you are legally able to enter into these Terms;
- you have been invited or otherwise authorized to use the service; and
- you use the service only for lawful purposes.
You are responsible for ensuring that your use of the service is permitted in your location and in your care setting.
3. Medical and emergency limitations
365CGM.com is not a medical device, a medical provider, or an emergency service. Does not produce or provide diagnostic, treatment, dosing, or advice recommendations or decisions. Information shown or transferred by 365CGM.com may at any time conflict with the source server or CGM, instructions from your clinician, device manufacturer, or official source systems. Always rely on those professional or official sources.
If you believe you may be experiencing a medical emergency, call emergency services immediately and follow the instructions of licensed clinicians and the manufacturers of your medical devices.
4. Your account
You are responsible for:
- keeping your account credentials confidential;
- ensuring the Eversense and any connected Nightscout information you provide is accurate;
- promptly reporting suspected unauthorized access; and
- logging out or requesting account disablement if you believe your account is compromised.
You may not share your account except as explicitly permitted by us.
5. Connected services
365CGM.com depends on third-party systems such as Eversense and, if configured, Nightscout. 365CGM.com requires users to authenticate with their own Senseonics User ID and password, as well as the CGM users', and with Nightscout to access their servers. An API is used to access and transfer CGM data. This information is stored temporarily for that purpose and may be modified or deleted at will using the Settings tab on the dashboard. The ID, passwords, and CGM data are never shared with any external service or used for any other purpose. The 365CGM.com website operates independently of the data transfer process once a Nightscout server is configured and online. Therefore, its webpage does not need to be displayed for the data transfer to be executed. This is an automatic process if the Senseonics and Nightscout servers are accessible and functioning. HTTPS (Hypertext Transfer Protocol Secure) encrypts communication between the user's browser and 365CGM.com, protecting data integrity and privacy.
We are not responsible for:
- the availability, security, accuracy, or uptime of third-party services;
- delays, gaps, or formatting changes in data returned by those services; or
- any actions taken by those providers regarding your account.

6. Mobile applications (365CGM Bridge)
365CGM.com offers a companion Android application, "365CGM Bridge," and an optional Wear OS watch component. Your use of those apps is governed by these Terms. A 365cgm.com account and a device token are required to use the app.
The app is a data-transfer utility. With your action and approval, it connects to diabetes devices you operate — NovoPen pens (NFC), Medtronic InPen pens (Bluetooth), Ascensia CONTOUR NEXT ONE and Keto-Mojo GK+ blood glucose meters (Bluetooth), and your approved Eversense CGM connection — and uploads their records to your 365cgm.com account and, if you configure it, to a Nightscout server you specify. Like the website, the app has no understanding of, and makes no claim to the accuracy of, the data it transfers, and it does not provide medical advice, diagnosis, treatment, or dosing recommendations.
You are responsible for keeping your device token confidential, for the accuracy and operation of the devices you connect, and for verifying any record against your original device and qualified clinical guidance before relying on it. NovoPen, InPen, Ascensia, CONTOUR, Eversense, Senseonics Holdings (Senseonics), Keto-Mojo, and Wear OS are trademarks of their respective owners; this app is an independent utility and is not affiliated with, endorsed by, or sponsored by those companies. The "as is," no-warranty, limitation-of-liability, and third-party- dependency terms elsewhere in this document apply to the app as they do to the website.
7. Acceptable use
You may not:
- use the service in a way that violates law or regulation;
- attempt to access other users' data;
- interfere with or disrupt the service;
- reverse engineer, scrape, or abuse the service in a way that harms availability or security;
- use the service to develop competing access paths or derivative infrastructure without permission; or
- use the service in any high-risk setting where failure could reasonably be expected to cause death, bodily injury, or material property damage.
8. Monitor links and sharing
If you select and copy the "Read-Only Monitor Link" found on your dashboard, you are responsible for deciding whether to share it and with whom. Anyone with the link can view the information it exposes until it is disabled, because the page is not password-protected. The function of this link will stop working upon the next page refresh, wherever desired, by clicking the "Disable Link" on the Monitor tab under Settings. Upon selecting and copying another link, a new, unique link is generated.
You are responsible for deciding whether to create, share, or revoke a monitor link. Treat monitor links as sensitive and be aware that if used on a public display device, the webpage may be cached, making it available to anyone and not deleted.
9. Service changes, suspension, and termination
We may change, suspend, or discontinue parts of the service at any time. We may disable or terminate access if we reasonably believe:
- your use violates these Terms;
- your account creates security, legal, or operational risk; or
- continued access could harm the service or others.
You may stop using the service at any time. Account deletion requests are handled through the product's account controls.
10. Data retention and deletion
We retain data in accordance with the Privacy Policy and operational needs, including security, troubleshooting, auditing, and synchronization. If you initiate an account deletion, we will attempt to delete or de-identify retained data in accordance with our systems and legal obligations.
11. No warranty
365CGM.com is provided on an "as is" and "as available" basis. We make no promise that the service will be uninterrupted, accurate, complete, timely, or suitable for any purpose.
To the maximum extent permitted by law, we disclaim all warranties, whether express, implied, or statutory, including warranties of merchantability, fitness for a particular purpose, title, non-infringement, accuracy, and uninterrupted availability.
We do not warrant that:
- glucose values, trends, or related events will always be accurate, current, complete, or timely;
- synchronization with Eversense or Nightscout will always succeed; or
- the service will be free of defects, downtime, or security incidents.
12. Limitation of liability
To the maximum extent permitted by law, 365 CGM and its operators will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for any loss of profits, data, goodwill, or business opportunity, arising out of or related to the service.
To the maximum extent permitted by law, our total liability for any claim arising out of or relating to the service will not exceed the greater of:
- the amount you paid us for the service in the 12 months before the claim; or
- US $100.
Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you.
13. Indemnity
You agree to defend, indemnify, and hold harmless 365 CGM and its operators from claims, liabilities, damages, losses, and expenses arising from your use of the service, your violation of these Terms, or your infringement of any rights of another person or entity.
14. Governing law and disputes
These Terms are intended to be interpreted under the laws designated by the service operator. If counsel has not yet specified a governing law or dispute process, the service operator may designate those terms in a future update.
15. Changes to these Terms
We may update these Terms from time to time. When we publish a new legal release, you may be required to review and accept the updated Terms before continuing to use the service.
