365 CGM Disclaimer

1. Not medical advice

365 CGM is an informational software tool. It does not provide medical advice, diagnosis, or treatment.

Nothing in the service should be interpreted as a substitute for professional medical judgment, manufacturer instructions, or advice from a licensed clinician.

2. Not for emergencies

Do not use 365 CGM for emergency or urgent medical decision-making.

If you think you may be having a medical emergency, call emergency services immediately and follow the instructions of qualified medical professionals.

3. Readings may be delayed, incomplete, or inaccurate

Glucose values, trends, event markers, and synchronization status may be delayed, missing, duplicated, stale, incomplete, or incorrect.

This can happen for many reasons, including:

  • delays or errors from Eversense or other connected services;
  • local or remote connectivity issues;
  • processing or synchronization delays;
  • software defects or outages; and
  • account or credential problems.

You must independently verify information before relying on it for any important decision.

4. Verify critical decisions independently

You are solely responsible for any action or inaction you take based on information shown in 365 CGM.

Before making treatment, dosing, safety, or other health-related decisions, verify the relevant information using the original device, official manufacturer systems, and qualified clinical guidance.

5. Third-party dependency limits

365 CGM depends on third-party systems such as Eversense and, if configured, Nightscout.

Those systems may change, fail, be unavailable, rate-limit requests, return incomplete information, or behave in ways that affect 365 CGM. 365 CGM does not control those systems and cannot guarantee their performance.

6. Shared monitor links are your responsibility

If you create a read-only monitor link, you are responsible for deciding whether to share it and with whom. Anyone with the link may be able to view the information exposed by that link.

7. No guarantee of uninterrupted or accurate service

365 CGM is provided on an "as is" and "as available" basis. We make no promise that the service will be uninterrupted, accurate, complete, timely, or suitable for any clinical, personal, or operational purpose.

8. Professional guidance comes first

If any information shown in 365 CGM conflicts with instructions from your clinician, device manufacturer, or official source systems, rely on those professional or official sources first.
