This Disclaimer covers the 365CGM.com website, the 365 CGM Android app and its Wear OS watch app, and the related tools (together, "365CGM", "our apps and tools"), operated by Prewitt Enterprises LLC. It is part of our Terms of Service (https://365cgm.com/legal/terms); our Privacy Policy (https://365cgm.com/legal/privacy) describes the data involved.
1. Not a medical device; no diagnostic, treatment, or advice recommendations or decisions
365CGM is not a medical device and does not diagnose, treat, cure, or prevent any medical condition. 365CGM.com and related apps are secondary-data or data-transfer utility tools to support and supplement the Eversense CGM and their tools, which are always primary. Our dashboard and apps act as a mirror for some but not all records retrieved from the Senseonics and Eversense devices and data servers for their FDA-approved medical device, the CGM Eversense 365. Our apps and tools have no understanding of, or accuracy in, the data or content they provide, display or transfer. They cannot and do not provide medical advice, diagnosis, or treatment.
Nothing in the 365CGM.com service or tools should be interpreted as a substitute for professional medical judgment, manufacturer instructions, or advice from a licensed clinician. Consult a healthcare professional for medical advice, diagnosis, or treatment. The service requires the external hardware and accounts it connects to — an Eversense CGM system and, optionally, supported meters and pens — and cannot function without them.
2. Not for emergencies
Do not use 365CGM.com or our apps for emergency or urgent medical decision-making. If you think you may be having a medical emergency, call emergency services immediately and follow the instructions of qualified medical professionals.
3. Readings may be delayed, incomplete, or inaccurate
Glucose values, trends, event markers, synchronization status, projections, statistics, or any data or content our tools provide may be delayed, missing, duplicated, stale, incomplete, or incorrect.
This can happen for many reasons, including:
- delays or errors from Eversense or other connected services;
- local or remote connectivity issues, including Bluetooth, NFC, Wi-Fi and cellular conditions;
- a transmitter that is charging, out of range, or connected to another app;
- processing or synchronization delays;
- software defects or outages; and
- account or credential problems.
You must independently verify information displayed or transmitted before relying on it for any important decision.
4. Verify critical decisions independently
You are solely responsible for any action or inaction you take based on information shown or transferred by 365CGM.com or our tools.
Before making treatment, dosing, safety, or other health-related decisions, verify the relevant information using the original device, official manufacturer systems, and qualified clinical guidance.
5. Third-party dependency and operational limits
365CGM.com depends on third-party systems such as Senseonics' Eversense services (the Eversense DMS and its remote-monitoring services) and, if configured, Nightscout. CGM users, and Eversense followers who want to see the person they follow, sign in to those services through 365CGM with their own Senseonics user ID and password; a Nightscout server needs its address and API secret. Signing in to our tools uses a password, and optionally a passkey or an authenticator app. Those credentials are stored encrypted with keys specific to your account, are used only to retrieve or deliver your data, and can be changed or removed at any time under Settings. How your data is shared — with the people you authorize, with Nightscout if you configure it, and with the service providers who run 365CGM — is described in the Privacy Policy.
With your credentials we read your data from Senseonics' systems; we do not write to them. Once your Eversense credentials and, optionally, a Nightscout server are configured, transfers run automatically on our servers whether or not a 365CGM page is open, for as long as the Senseonics and Nightscout servers are reachable and working. HTTPS encrypts communication between your browser, the app, the watch and 365CGM.com. CGM and other data may be read from or sent to your phone, watch or other connected devices by Bluetooth, NFC, Wi-Fi or cellular connections.
The 365 CGM app and the watch app can read your Eversense transmitter directly over Bluetooth. Only at your direction, the app can also send two things to the transmitter: a fingerstick calibration value you enter, and the transmitter's vibration (do-not-disturb) setting. It does not otherwise change the transmitter, its sensor, or Senseonics' servers. How the apps share the transmitter with the Eversense app and other devices, and what happens if you run them without the Eversense app, is in Section 6.

Those systems may change, fail, be unavailable, rate-limit requests, return incomplete information, or behave in ways that affect the data. 365CGM.com does not control those systems and cannot guarantee their performance.
6. How the apps work, what they do for you, what you can configure, and working with or without the Eversense app
In short. 365CGM takes the glucose your Eversense transmitter is already measuring and puts it where you want it — on your phone, your watch, your lock screen, a home-screen widget and a web dashboard — and adds what the Eversense app does not collect: fingerstick and ketone readings from your meters, injections from your smart pens, the medications you take, your calibrations, notes and events, all in one record that you can share with the people you choose. It is designed and built to work alongside the Eversense 365 app and your other devices, and it does so in most normal configurations.
In more detail. Depending on what you connect and turn on, the service gives you:
- Your glucose, everywhere. The current value, trend and a live chart on the website, in the app, on the watch face, in a watch tile and app, in an optional floating bubble, on the lock screen, and in a home-screen widget; readings arrive from the transmitter directly, from the Eversense app's notification (companion mode), or from Senseonics' servers, whichever you use, and are merged into one record.
- Blood glucose and ketones from your meters. Automatic capture of fingerstick readings from a CONTOUR NEXT ONE and glucose and ketone readings from a Keto-Mojo GK+ over Bluetooth, in the background if you wish, with an optional spoken confirmation, plus manual entry on the website and in the app.
- Injections and medications. Every dose recorded by a NovoPen (by NFC tap) or an InPen (by Bluetooth), with the insulin assigned to each pen, pen start dates, and dose logging from the wrist; a medication catalog, your own medication list, and meal-time groups so a whole routine is logged with one tap; all of it shown on the chart next to your glucose.
- Better-timed calibrations. A dashboard indicator that tells you when your glucose is steady enough for an Eversense calibration; calibration entered from the app or the watch — or from the website, with the phone carrying it out — and sent straight to the transmitter, with the fingerstick recorded at the same time; a calibration history and status; a reminder for a calibration you chose to defer; and a sensor-lag measurement (passive, or a short guided session) that anchors the projection line to your own physiology.
- Transmitter alignment. A live signal-strength meter read from the transmitter — started from the phone, the watch or the website — so you can position it over the sensor for the best coupling, with the transmitter's battery shown alongside.
- A watch that takes over. The watch normally shows what the phone relays, but if the phone is off, out of range or not reading, the watch reads the transmitter itself, uploads with its own token, and hands back when the phone returns — decided once per five-minute cycle and designed so that a single miss costs no record. Heart rate, steps and activity from the watch can appear on your dashboard if you allow it.
- Alarms and projections. Low, high, urgent, "low soon" / "high soon" and stale-data alarms with your own thresholds, sounds, repeat and duration, on the website, in the app, on the watch and as browser push notifications; a projection line that forecasts 5–30 minutes ahead and learns from its own results (accuracy shown as MARD); and the transmitter's own on-body vibration alerts switched from the website.
- Sharing. Followers, family and medical providers see your dashboard on their own accounts with the permissions you set feature by feature; Eversense followers get live data by signing in with their own Eversense credentials; a read-only share link needs no account; a built-in Nightscout-compatible feed (nightscout.365cgm.com, protected by a secret key you can change or turn off) lets xDrip+, Nightscout clocks and similar displays show your live glucose; and, if you run one, every reading is delivered to your own Nightscout server.
- History, reports and statistics. Time in range, detailed statistics, sensor accuracy and drift, long history with a one-year import of older readings and events, Eversense events (meals, exercise, health) on the chart, your notes, and printable reports.
- Your data, your control. Export and import of your configuration and chart data, encrypted daily backups, sign-in with passkeys or an authenticator app, device tokens you create and revoke, multiple phones under one account with each meter or pen captured by one phone at a time, mg/dL or mmol/L, light and dark themes, US and outside-US Eversense accounts, and account deletion in one place.
- Ask AI and Help. Questions about your own data answered from what is on your dashboard.
- Any device with a browser. The website — dashboard, settings, sharing, reports — works in a current browser on an iPhone or iPad, a Mac or Windows PC, a tablet, and the browser built into some cars and televisions, and it can be installed like an app on a phone or tablet; the read-only share link opens on any of them without an account. What a browser can and cannot do is set out below.
How the data flows. Your Eversense transmitter measures glucose from the sensor and keeps a record of its readings. Senseonics' Eversense app reads the transmitter and stores the readings on Senseonics' servers, and the 365CGM.com website reads them from there with your Eversense credentials. The 365 CGM app and the watch app add a second path: they can read the transmitter directly over Bluetooth, or, in companion mode, read the value the Eversense app shows in its own notification, and upload what they read to your 365CGM.com account. Your account merges the readings from every path, keeps them, and shows and shares them the way you have set up. Meter and pen records are collected the same way and uploaded to your account and, if configured, to Nightscout. The apps only relay what the transmitter and your devices report, plus the entries you type in yourself; they do not invent or correct readings.
What you configure. You decide how the apps run: which meters, pens and transmitter to connect; whether the phone reads the transmitter directly or uses companion mode; automatic upload or a review mode in which you approve each record; whether capture keeps running in the background; whether the watch only shows what the phone relays or also takes over when the phone is out of reach; which alarms sound, how, and on which device; the floating bubble, lock-screen display and widget; who can see or change your data; delivery to a Nightscout server; and whether you enter calibrations through our app. Every one of these can be changed or turned off at any time in the app, on the watch, or under Settings on the website, and each choice changes what is captured and where it is shown.
On other internet-connected devices — iPhone and iPad, computers, tablets, cars and TVs. The website works in any current browser on any device with an internet connection, and can be installed like an app (Android: the Install button; iPhone and iPad: Share → Add to Home Screen). The 365 CGM app and the watch app, however, are Android and Wear OS 3.0+ only. An iPhone, iPad, Apple Watch, Fitbit or Garmin, a car, or a television cannot read your transmitter, meters or pens through 365CGM; on those devices your own CGM data reaches 365CGM only through Senseonics' servers (the Eversense app on your phone → their servers → our website), usually a few minutes behind the transmitter, and meter and pen records must come from an Android phone. There is no Android Auto or Apple CarPlay app: in a car, use your phone or watch, or the website or a share link in the car's own browser if it has one — never while driving. Browser limits apply on every such device: a browser will not play an alarm sound until you have tapped the page once; the share-link page shows alarms visually only, without sound; notifications on an iPhone or iPad need iOS 16.4 or later and the site installed to the Home Screen; a browser that is closed, or a tab the device has put to sleep, raises no alarm; and a car or television browser may stop refreshing while the vehicle is moving or the screen is off. Without an internet connection the website cannot update; the app keeps showing its last readings and keeps the records it captures from your meters and pens for upload when the connection returns; the watch can read the transmitter without the phone but needs the phone, Wi-Fi or its own cellular connection to upload.
Working alongside the Eversense app and your other devices. Our service is designed and built to work with these devices and to minimize conflicts, and it works in most normal configurations. It is not always possible to avoid a conflict or a delay in transferring or viewing data, because the transmitter allows one app session at a time: while the Eversense app and the 365 CGM app (or the watch) are both connected, they take turns on that session, and a reading in either app can occasionally be delayed, interrupted or reconnected, and in rare cases one app may need to be closed for the other to read. Other devices that talk to the transmitter or to the Eversense app — an insulin pump, a receiver, or another connected device — share the same constraint, and we cannot promise that our apps work with every such device. Bluetooth meters and pens usually pair with one phone at a time, so a device paired to another phone reports to that phone.
Working without the Eversense app. Our apps can read the transmitter and run without the Eversense app installed or running, and some people prefer that. If you choose it, your readings are still captured and kept in your 365CGM.com account, but nothing is written to Senseonics' servers while the Eversense app is not running: your Eversense followers, clinic or remote-monitoring service see no new data, the Eversense app's own alarms, calibration reminders and features are unavailable, and the transmitter receives no firmware updates. When you run the Eversense app again, the transmitter backfills the readings it has kept to that app and to Senseonics' servers, and the gap on their side fills in to the extent the transmitter kept them. 365CGM keeps what it captured in the meantime.
Advantages and disadvantages. Reading the transmitter with our apps gives you your glucose on the watch, the phone and the dashboard, sharing with the people you choose, and capture that does not depend on the Eversense app or on Senseonics' servers being up. Running the Eversense app alongside keeps Senseonics' record, remote monitoring, alarms and firmware current at the cost of two apps sharing one transmitter. Running both is the arrangement most people use, and the one the service is built for. Which arrangement suits you depends on your needs and preferences; the choice, and the consequences of the choice, are yours. If in doubt, keep the Eversense app running and treat our apps as the supplement they are — the Eversense app and Senseonics' systems are always primary and hold the official record.
7. Shared links and shared access are your responsibility
A share link (Settings → Share → "Copy share link") opens a read-only, limited view of your dashboard. You are responsible for deciding whether to share it and with whom: anyone with the link can view the information it exposes until it is disabled, because the page is not password-protected. You can stop the link at any time with "Disable link" on the same card; it stops working at the next page refresh. Copying a new link generates a new, unique link and retires the old one.
Treat share links as sensitive and be aware that if used on a public display device, the webpage may be cached, making it available to anyone and not deleted.
The built-in Nightscout-compatible feed (Settings → Nightscout → Local Nightscout Server) works the same way: while it is enabled, anyone who has its Read URL and secret key can read your current glucose and trend from any Nightscout-compatible reader. Keep the key private, change it with "New secret" if it may have leaked, and press Disable to stop publishing.
The same applies to the people you grant access to under Settings → Share — followers, family members, other users and medical-provider accounts. You choose, feature by feature, whether each of them sees nothing, sees only, or can see and modify records in your account, and you can change or cancel that at any time. We are not responsible for what someone you chose to share with does with your information.
8. The 365 CGM app and the Wear OS watch app
The app and its optional Wear OS watch app are data-transfer utilities, the same as the website. The app collects insulin-dose records, selected health data and blood-glucose and ketone readings from devices you operate and connect yourself (NovoPen pens by NFC tap; Medtronic InPen pens, Ascensia CONTOUR NEXT ONE meters and Keto-Mojo GK+ meters by Bluetooth), reads or relays Eversense CGM data through your approved connection — the transmitter directly, or, in optional companion mode, the glucose value shown in the Eversense app's own notification — and uploads those records to your 365cgm.com account and any Nightscout server you configure.
These records may be delayed, missing, duplicated, stale, incomplete, or incorrect for the same reasons described above — device, connectivity, synchronization, or software conditions, including the phone's battery, background-activity and notification settings and a capture service that has been stopped. The apps have no understanding of, and make no claim to the accuracy of, the data they transfer, and do not provide medical advice, diagnosis, treatment, or dosing decisions. Always verify a dose or reading against the original device, official manufacturer systems, and qualified clinical guidance before relying on it. A glucose value, trend or other data shown on a Wear OS watch or other device is relayed from your phone or other data collector for convenience — or, when the watch reads a transmitter paired to it, is the same transmitter record the phone would read — and is not an independent measurement. Heart rate, steps and activity from the watch are wellness information from the watch's own sensors, not medical measurements.
9. Alarms and notifications are not a safety system
The website, the app and the watch can raise glucose, trend, stale-data and device alarms and send notifications according to the settings you choose. An alarm can be late or missed because of connectivity, a data gap, your phone's battery, notification, do-not-disturb or background-activity settings, a closed browser or a paused service. Our alarms are not a substitute for the alarms of your Eversense system. Never rely on 365CGM alone to warn you of a low or high glucose.
10. Ask AI and Help answers are generated automatically
Answers from the Ask AI and Help features are produced by third-party AI models from your question and the data on your dashboard. They can be wrong, incomplete or out of date, and are not medical advice, diagnosis, treatment or dosing guidance. Verify anything they say against your devices, official sources and qualified clinical guidance before acting on it.
11. No guarantee of uninterrupted, continued, free or accurate service
365CGM.com and tools are provided on an "as is" and "as available" basis. We make no promise that the service will be uninterrupted, accurate, complete, timely, or suitable for any purpose. The service, or any of its functions, may be changed, suspended or discontinued at any time and without notice, and it may not remain free of cost or charges (see the Terms of Service).
12. Professional guidance and data from the Eversense CGM must be used first
Information shown or transferred by 365CGM.com or our tools may at any time conflict with the source server or CGM, instructions from your clinician, device manufacturer, or official source systems. Always rely on those professional or official sources first.

The Eversense® 365 Continuous Glucose Monitoring (CGM) System is indicated for continually measuring glucose levels for up to one year in people (18 years and older) with diabetes. The system is indicated for use to replace fingerstick blood glucose (BG) measurements for diabetes treatment decisions.
