Create your 365CGM.COM account

365CGM.COM works with any of these consumer account types. CGM Users and Followers sign in with their existing Eversense account; an Open Account needs no Eversense account at all:

  • CGM User (Eversense 365 app, signs in at us.eversensedms.com) — you wear the sensor.
  • Follower (Eversense Now app) — you monitor someone else's sensor; they must have invited you and you must have accepted in the Eversense Now app.
  • Open Account — you don't wear a sensor and aren't following anyone yet, and you don't need an Eversense account. Just pick an email and password below. To receive and view someone's CGM data later, they grant you access and you accept their invitation.
  • Medical Provider Office — for clinics and practices: follow one or more 365CGM.COM users with read access plus provider notes. No Eversense account needed; each patient approves your access.

If you're a CGM User or a Follower, enter your existing Eversense email and password below — we'll detect which kind it is. If you don't have an Eversense account yet, create one at eversensedms.com first. (Open Accounts can skip this.)

Not supported: Eversense DMS Pro accounts (uspro.eversensedms.com, for healthcare professionals) use a separate API and won't work here.

Your 365CGM.COM sign-in

For the highest security, pick the email and password you'll use to sign in to 365CGM.COM. Our password requirements are stronger than Eversense's. However, you may use the same email and password as your Eversense account below — that way you only have one set of credentials to remember; except if your Eversense password is shorter than 12 characters, you must pick a stronger one here.

12+ characters with at least one letter, one number, and one special character (!@#$%^&*()_+-=[]{}|;:,.<>?~).

Your Eversense account credentials

Enter YOUR OWN Eversense login — the same email and password you use in your Eversense app on your phone. If you picked "I'm a follower" above, this is your follower account (not the CGM user's). If you picked "I'm the CGM user", this is your CGM-user account. We store the password encrypted and use it only to fetch your CGM data; it's kept separate from your 365CGM.COM password above.

Eversense region

Pick the Eversense region your account is registered with. You can change this later.

Your patient on 365CGM.COM

Enter the email your patient uses to sign in to 365CGM.COM. We'll email them your follow request with accept/reject links; once they accept you get view-only access plus provider notes. You can request more patients later from Settings → Share.
